A.R.S. 12-601 — an application in the superior court of the county of residence

A person who wants to change names files an application in the superior court of the county of residence, giving the reasons and the name wanted, and the court may enter judgment substituting the new name. For a minor, a parent, guardian ad litem or next friend applies in the minor's county, and the court considers the minor's best interests.

The applicant states under penalty of perjury whether the applicant has been convicted of a felony, whether certain felony charges, including charges involving false statements about identity, are pending, and whether the change is to another person's name to commit or further an offense.
