Idaho Code § 39-4510 — an advance care planning document, by anyone eighteen or older
Any competent person aged eighteen or older may make an advance care planning document. It is valid if it includes the person's name, date of birth, phone number and mailing address, the signature of the person or an authorized agent, and the date signed. Blanks left are treated as intentional.
- Naming one or more health care agents, with their contact details and relationship.
- Resuscitation instructions, and instructions on pregnancy.
- Instructions on releasing health information under federal privacy law.
- Instructions for end-of-life care and a description of treatment objectives.
Those are among what the document may include. The state health department may publish an optional form, and it is not mandatory.
