Changing A Name
N.J.S.A. 2A:52-1 — a complaint in Superior Court with a sworn affidavit
In New Jersey, any person may bring an action in Superior Court for authority to take another name. The complaint comes with a sworn affidavit giving the person's name, date of birth and social security number, any criminal convictions or pending charges, and a statement that the change is not meant to avoid prosecution or creditors or to commit fraud.
Knowingly giving false information in the process is a crime of the fourth degree. With charges pending, the complaint is also served on the prosecutor.
