Changing a Name
La. R.S. 13:4751 — an adult petitions the district court of the parish of residence or birth
An adult who wants a new name petitions the district court of the parish of residence, the parish of birth, or the parish of venue for the Vital Records Registry.
- A minor's petition is signed by both parents, or the survivor if one has died.
- A parent with court awarded custody may proceed without the other parent in the circumstances the law lists, such as where the other parent's rights were terminated.
- A person convicted of a felony may not petition until the sentence is satisfied, including probation or parole.
- A person convicted of a crime of violence may not petition at all.
