15 V.S.A. § 811 — a signed, acknowledged instrument before the probate judge where the person lives

A person of age and sound mind changes their name by making, signing, sealing and acknowledging an instrument before the judge of the Probate Division in the district where they live, in the form the section sets out.

On granting a divorce, the court allows a spouse to resume a prior name, or the name of a former spouse, unless good cause is shown otherwise.
