Fla. Stat. 717.124 — a verified claim with identification, and a claimant representative must be registered
In Florida, a person claiming an interest in property paid or delivered to the department may file a claim on the department's form, verified by the claimant or a claimant representative registered with the department.
Identification with the claim. The claim includes a legible copy of the claimant's valid driver license, or other photographic identification, or a notarized sworn statement of identity. A claim filed without it, or without any required recovery or purchase agreement, is void.
Errors and withdrawal. Within 90 days the department may return a claim that seeks fees above what the chapter allows or has apparent errors, and a claim is withdrawn if the claimant does not answer a written request for information within 60 days.
